事故調查組結合消防和專家意見以及事故調查實際情況，認定事故直接原因是由于1-己烯灌裝重量設定值設置錯誤，導致原設定充裝140kg的物料桶灌裝至130kg左右時1-己烯氣液相溢出并發生噴濺產生靜電釋放發生閃燃。
Based on the opinions of the Fire and Rescue Detachment and experts as well as the real conditions of the investigation, the Accident Investigation Team identified that the accident was directly caused by the inaccurate set value of filling weight of 1-Hexene, and consequently when the material barrel originally set to fill 140kg was filled to about 130kg, 1-Hexene gas spilled at liquid phase and splashed, generating electrostatic discharge and leading to flashover.

2. Indirect cause
 
存在違規作業現象。違反《關于進一步加強夏季危險化學品道路運輸作業安全管理的通知》（滬安監管?；?011〕105號）第四條關于“夏季危險化學品禁止在上午8時至下午4時進行裝卸作業”的規定，在禁止裝卸作業的時段內進行危險化學品1-己烯的卸料灌裝作業，間接導致事故發生。
Existence of rule-breaking operation. The Company violated the provision of Article 4 of the Notice on Further Enhancing the Safety Management of Road Transportation of Hazardous Chemicals in Summer (H. A. J. G. W. H. [2011] No. 105), “the handling operation of hazardous chemicals is forbidden from 8:00am to 4:00pm in summer”, and the unloading and filling operation of hazardous chemical 1-Hexene at the forbidden time frame indirectly caused the accident.
